ABOUT THE SPEAKER
Andrew Bastawrous - Eye surgeon, inventor
Andrew Bastawrous studies eye health -- and builds accessible new tools to bring eye care to more people.

Why you should listen

Andrew Bastawrous is cofounder and CEO of Peek Vision, a social impact organization that uses smartphone technology to radically increase access to eye care in some of the most challenging places in the world. Bastawrous is also an ophthalmologist (eye surgeon) and Associate Professor in International Eye Health at the London School of Hygiene and Tropical Medicine.

Peek's vision app, Peek Acuity, is being used to transform eye health in more than 150 countries. The organization ran one of the top-ten all-time Indiegogo crowd-funding campaigns in the health sector -- for Peek Retina, a smartphone ophthalmoscope -- and was voted "best social-impact start-up in Europe" by Google and McKinsey in 2016. All prize money has been re-invested in Peek.

Bastawrous is working with astrophysicists to crowdsource retinal data; with National Geographic explorers to reach isolated communities; and with artists and activists to advocate for eye care globally. He and his wife Madeleine have established a social enterprise healthy bakery in Kenya that provides employment, with profits paying for eye care.

In 2018, Bastawrous was invited to deliver the Commonwealth Address in the presence of the British Royal Family, where he announced the creation of a new one-billion-dollar Vision Catalyst Fund -- a new vision for international aid.

we moved to Kenya.

We secured equipment,

Conseguimos equipamentos,

funds, vehicles, we trained a team,

00:45
to try and understand a single question:
14
33190
3461
para tentar entender uma única questão:
00:48
why are people going blind,
15
36651
2086
por que as pessoas estão ficando cegas,
00:50
and what can we do?
16
38737
2919
e o que podemos fazer?
00:53
The challenges were great.
17
41656
2914
Os desafios eram grandes.
00:56
When we got to where we were going,
18
44570
1831
Quando chegamos aonde estávamos indo,
00:58
we set up our high-tech equipment.
19
46401
2666
preparamos nosso equipamento
de alta tecnologia.
01:01
Power was rarely available.
20
49067
2179
Raramente tínhamos energia.
01:03
We'd have to run our equipment
21
51246
853
Tínhamos que usar os equipamentos
com geradores a gasolina.
01:04
from petrol power generators.
22
52099
2900
Até que uma coisa me veio à mente:
01:06
And then something occurred to me:
23
54999
1947
01:08
There has to be an easier way,
24
56946
2495
Tem que haver um jeito mais fácil,
01:11
because it's the patients who are the most in need
25
59441
2000
porque são os pacientes que mais precisam
01:13
of access to eye care
26
61441
1558
de atendimento oftalmológico
01:14
who are the least likely to get it.
27
62999
2341
que têm menos chances de consegui-lo.
01:17
More people in Kenya,
28
65340
1106
Mais pessoas no Quênia,
01:18
and in sub-Saharan Africa,
29
66446
1912
e na África Subsaariana,
01:20
have access to a mobile phone
30
68358
1791
têm acesso a um telefone celular
01:22
than they do clean running water.
31
70149
2153
do que a água limpa e corrente.
01:24
So we said, could we harness
32
72302
1719
Será que podemos aproveitar
o poder da tecnologia dos celulares
01:26
the power of mobile technology
33
74021
1619
para dar atendimento
oftalmológico de um jeito novo?
01:27
to deliver eye care in a new way?
34
75640
2151
01:29
And so we developed Peek,
35
77791
1785
E assim desenvolvemo o Peek,
01:31
a smartphone [system] that enables
36
79576

01:34
community healthcare workers

82114
1512
funcionários de saúde da comunidade
01:35
and empowers them to deliver eye care everywhere.
38
83626

e os capacita a dar atendimento
oftalmológico em qualquer lugar.
01:39
We set about replacing
traditional hospital equipment,
39
87456
2670
Começamos a substituir
equipamentos hospitalares tradicionais
01:42
which is bulky, expensive and fragile,
40
90126
2523
que são grandes, caros e frágeis,
01:44
with smartphone apps and hardware
41
92649
2035
por aplicativos para
smartphones e hardware
01:46
that make it possible to test anyone
42
94684
2137
que permitem testar qualquer pessoa
01:48
in any language and of any age.
43
96821
3189
em qualquer idioma e de qualquer idade.
01:52
Here we have a demonstration
44
100010
1804
Aqui temos uma demostração
01:53
of a three-month-old having their vision
45
101814
1723
de um bebê de três meses passando
01:55
accurately tested using an app and an eye tracker.
46
103537
4373
por um teste de visão preciso, usando
um aplicativo e um rastreador ocular.
02:03
We've got many trials going on
47
111650
1743
Estamos fazendo vários testes
02:05
in the community and in schools,
48
113393
2200
na comunidade e nas escolas,
02:07
and through the lessons that
we've learned in the field,
49
115593
1900
e com as lições que aprendemos no campo,
02:09
we've realized it's extremely important
50
117493
1703
percebemos que é muito importante
02:11
to share the data in non-medical jargon
51
119196
2860
passar a informação sem termos médicos
02:14
so that people understand
52
122056
1661
para que as pessoas entendam
02:15
what we're examining and what that means to them.
53
123717
2623
o que estamos examinando
e o que isso significa.
02:18
So here, for example,
54
126340
1274
Aqui, por exemplo,
02:19
we use our sight sim application,
55
127614
1856
usamos nosso aplicativo Sight-Sim
02:21
once your vision has been measured,
56
129470
1792
depois que sua visão foi medida,
02:23
to show carers and teachers
57
131262
2710
para mostrar aos pais e professores
02:25
what the visual world is like for that person,
58
133972
2514
como aquela pessoa enxerga o mundo,
02:28
so they can empathize with them and help them.
59
136486
3466
para que possam entender
como se sentem e ajudá-los.
02:31
Once we've discovered somebody has low vision,
60
139952
2388
Uma vez descoberto
que alguém tem baixa visão,
02:34
the next big challenge is to work out why,
61
142340
2571
o próximo desafio é entender por quê,
02:36
and to be able to do that,
62
144911
1117
e para ser capaz de fazer isso,
02:38
we need to have access to the inside of the eye.
63
146028
2501
precisamos ter acesso ao interior do olho.
02:40
Traditionally, this requires expensive equipment
64
148529
2586
Tradicionalmente, seria necessário
um equipamento caro
02:43
to examine an area called the retina.
65
151115
2081
para examinar uma área chamada retina.
02:45
The retina is the single part of the eye that has
66
153196
2809
A retina é a parte única do olho que tem
02:48
huge amounts of information about the body
67
156005
2096
muita informação sobre o corpo
02:50
and its health.
68
158101
1788
e sua saúde.
02:51
We've developed 3D-printed,
69
159889
1811
Desenvolvemos hardware
de baixo custo impresso em 3D
02:53
low-cost hardware
70
161700
1441
02:55
that comes in at less than five dollars to produce,
71
163141
2400
que pode ser produzido
por menos de cinco dólares,
02:57
which can then be clipped onto a smartphone
72
165541
2943
que pode ser acoplado a um smartphone
03:00
and makes it possible to get views
73
168484
1442
e possibilita obter uma visão
03:01
of the back of the eye
74
169926
1221
da parte preta do olho
03:03
of a very high quality.
75
171147
1756
com uma qualidade altíssima.
03:04
And the beauty is, anybody can do it.
76
172903
3197
E a beleza disso é que
qualquer um pode fazer.
03:08
In our trials on over two and half thousand people,
77
176100
2879
Em nossos testes em mais de 2.500 pessoas,
03:10
the smartphone with the add-on clip
78
178979
1917
o smartphone com a modificação
03:12
is comparable to a camera
79
180896
1681
se compara a uma câmera
03:14
that is hugely more expensive
80
182577
1895
que é imensamente mais cara
03:16
and hugely more difficult to transport.
81
184472
3589
e imensamente mais difícil de transportar.
03:20
When we first moved to Kenya,
82
188061
1879
Quando nos mudamos para o Quênia,
03:21
we went with 150,000 dollars of equipment,
83
189940
2653
fomos com 150 mil dólares em equipamentos,
03:24
a team of 15 people,
84
192593
2457
uma equipe de 15 pessoas,
03:27
and that was what was
needed to deliver health care.
85
195050
2892
e era disso que precisávamos
para oferecer atendimento médico.
03:29
Now, all that's needed
86
197942
2365
Agora, tudo o que precisamos
03:32
is a single person
87
200307
1792
é uma única pessoa
03:34
on a bike with a smartphone.
88
202099
3396
em uma bicicleta e um smartphone.
03:37
And it costs just 500 dollars.
89
205495
3854
E só custa 500 dólares.
A questão da energia elétrica
03:41
The issue of power supply
90
209349
1870
foi superada aproveitando-se
a energia solar.
03:43
is overcome by harnessing the power of solar.
91
211219
2561
Nossos funcionários da saúde viajam
com uma mochila a energia solar.
03:45
Our healthcare workers travel
92
213780

93
214920

03:48
which keeps the phone charged and backed up.
94
216465
2461
que mantém o celular carregado e seguro.
03:50
Now we go to the patient
95
218926
2270
Agora nós vamos ao paciente,
03:53
rather than waiting for the patient never to come.
96
221196
2711
em vez de esperar que ele nunca venha.
03:55
We go to them in their homes
97
223907
1733
Vamos até eles em suas casas
03:57
and we give them the most comprehensive,
98
225640
1797
e oferecemos os exames
03:59
high-tech, accurate examination,
99
227437

mais abrangentes, precisos
e de alta tecnologia
04:01
which can be delivered by
anyone with minimal training.
100
229377
4104
que podem ser feitos por qualquer um
com um treinamento mínimo.
04:05
We can link global experts
101
233481
2837
Podemos conectar especialistas globais
04:08
with people in the most rural,
102
236318
1742
a pessoas nos lugares mais rurais
e de difícil acesso
04:10
difficult-to-reach places
103
238060
1190
que estão além do fim da estrada,
04:11
that are beyond the end of the road,
104
239250
1589
04:12
effectively putting those experts in their homes,
105
240839
2514
colocando efetivamente
esses especialistas em suas casas,
04:15
allowing us to make diagnoses
106
243353
1978
permitindo-nos dar diagnósticos
04:17
and make plans for treatment.
107
245331
3068
e fazer planos para o tratamento.
04:20
Project managers, hospital directors,
108
248399
2727
Gerentes de projeto,
diretores de hospitais,
04:23
are able to search on our interface
109
251126
1919
podem pesquisar em nossa interface
04:25
by any parameter they may be interested in.
110
253045
2002
por qualquer parâmetro que quiserem.
04:27
Here in Nakuru, where I've been living,
111
255047
2023
Aqui em Nakuru, onde estou morando,
04:29
we can search for people
112
257070
1336
podemos pesquisar por pessoas
por qualquer condição.
04:30
by whatever condition.
113
258406
1444
04:31
Here are people who are blind
114
259850
1272
Aqui são pessoas que ficaram cegas
04:33
from a curable condition cataract.
115
261122
2530
de uma condição de catarata curável.
04:35
Each red pin depicts somebody
116
263652
2029
Cada ponto vermelho representa alguém
04:37
who is blind from a disease that is curable
117
265681
2657
que ficou cego de uma doença curável
04:40
and treatable, and they're locatable.
118
268338
2246
e tratável, e são localizáveis.
04:42
We can use bulk text messaging services
119
270584
1884
Usamos serviços de mensagens em massa
04:44
to explain that we're coming to arrange a treatment.
120
272468
3154
para explicar que vamos
arranjar um tratamento.
04:47
What's more, we've learned that this is something
121
275622
2615
Além disso, aprendemos que isso é algo
04:50
that we haven't built just for the community
122
278237
2038
que não construímos só para a comunidade
04:52
but with the community.
123
280275
1585
mas com a comunidade.
04:53
Those blue pins that drop represent
124
281860
2252
Aqueles pontos azuis que diminuem
04:56
elders, or local leaders,
125
284112
2101
representam idosos, ou líderes locais,
04:58
that are connected to those people
126
286213
1239
que se conectam a essas pessoas
que garantem que os encontremos
04:59
who can ensure that we can find them
127
287452
1333
05:00
and arrange treatment.
128
288785
2101
e arranjemos tratamentos.
05:02
So for patients like Mama Wangari,
129
290886
3002
Então para paciente como Mama Wangari,
05:05
who have been blind for over 10 years
130
293888
1596
que ficou cega há mais de 10 anos
e nunca viu seus netos,
05:07
and never seen her grandchildren,
131
295484
1629
05:09
for less than 40 dollars, we can restore her eyesight.
132
297113
3818
por menos de 40 dólares,
podemos restaurar sua visão.
05:12
This is something that has to happen.
133
300931
5278
Isso é algo que tem que acontecer.
05:18
It's only in statistics
134
306209
1631
É só na estatística
05:19
that people go blind by the millions.
135
307840
2723
que as pessoas ficam cegas aos milhões.
05:22
The reality is everyone goes blind on their own.
136
310563
3868
Na realidade, cada um fica cego por si só.
05:26
But now, they might just be
137
314431
1977
Mas agora, eles podem estar
05:28
a text message away from help.
138
316408
2139
a uma mensagem de conseguir ajuda.
05:30
(Applause)
139
318547
5936
(Aplausos)
E porque demonstrações ao vivo
são sempre má ideia,
05:37
And now because live demos are always a bad idea,
140
325295
2131
05:39
we're going to try a live demo.
141
327426
1798
nós vamos fazer uma.
05:41
(Laughter)
142
329224
1256
(Risos)
05:42
So here we have the Peek Vision app.
143
330480
4685
Aqui nós temos o aplicativo Peek Vision.
05:52
Okay, and what we're looking at here,
144
340142
2644
Certo, e o que estamos vendo aqui,
05:54
this is Sam's optic nerve,
145
342786
1495
este é o nervo ótico de Sam,
05:56
which is a direct extension of her brain,
146
344281
2399
que é uma extensão direta de seu cérebro,
05:58
so I'm actually looking at her brain as we look there.
147
346680
2609
então estamos vendo
seu cérebro ao olhar ali.
06:01
We can see all parts of the retina.
148
349289
3531
Podemos ver todas as partes da retina.
06:04
It makes it possible to pick up diseases
149
352820
2530
Torna possível detectar doenças
06:07
of the eye and of the body
150
355350
2167
do olho e do corpo,
06:09
that would not be possible without access to the eye,
151
357517
2529
o que não seria possível
sem acesso ao olho,
06:12
and that clip-on device can be manufactured
152
360046
1935
e esse dispositivo anexado
pode ser fabricado
06:13
for just a few dollars,
153
361981
1550
por apenas alguns dólares,
06:15
and people can be cured of blindness,
154
363531
1631
e pessoas podem ser curadas da cegueira,
e acho que isso diz muito
sobre nós como raça humana,
06:17
and I think it says a lot about us as a human race
155
365162
