ABOUT THE SPEAKER
Mark Kendall - Biomedical engineer
Mark Kendall aims to shake up how vaccines are delivered with the Nanopatch.

Why you should listen

Looking a bit like a fuzzy computer chip, the Nanopatch uses tiny powder-coated spikes to deliver a small dose of vaccine just under the skin, immunizing a person in about a minute. Made for less than $1, it uses only a fraction of a vaccine dose delivered by traditional syringe method (which was invented in 1853), at the same time eliminating the risk of needle injuries. What’s more, a Nanopatch infused with vaccine is designed to be heat-stable, so it can be transported without refrigeration. And the process doesn't draw blood, reducing the risk of infections.

Mark Kendall, an Australian biomedical engineer, was part of a team at the University of Queensland that advanced the Nanopatch by vaccinating animals. Now his company, Vaxxas, is on a mission to commercialize the device for human use. He plans to run an international trial using the Nanopatch, starting with the human papilloma virus (HPV) vaccine to protect against cervical cancer.

02:10
One is it could be holding保持 back

02:54
what you see are thousands数千 of tiny projections预测

02:56
that are invisible无形 to the human人的 eye.
62
164533
2221
02:58
And there's about 4,000 projections预测
63
166754
1757
03:00
on this particular特定 square广场 compared相比 to the needle.
64
168511
3382
03:03
And I've designed设计 those projections预测
65
171907
2644
03:06
to serve服务 a key role角色, which哪一个 is to
work with the skin's皮肤的 immune免疫的 system系统.
66
174551
4050
03:10
So that's a very important重要 function功能
67
178601
2395
03:12
tied in with the NanopatchNanopatch.
68
180996
1456
03:14
Now we make the NanopatchNanopatch
69
182452
2247
03:16
with a technique技术
70
184699
2537
03:21
And this particular特定 technique技术
is one that's been borrowed
72
189615
2044
03:23
from the semiconductor半导体 industry行业,
73
191659
1627
03:25
and therefore因此 is low cost成本
74
193286
1836
03:27
and can be rolled热轧 out in large numbers数字.
75
195122
2194
03:29
Now we dry-coat干涂层 vaccines疫苗 to
the projections预测 of the NanopatchNanopatch
76
197316
5066
03:34
and apply应用 it to the skin皮肤.
77
202382
1819
03:36
Now, the simplest简单 form形成 of application应用
78
204201
4556
03:40
is using运用 our finger手指,
79
208757
2022
03:42
but our finger手指 has some limitations限制,
80
210779
2620
03:45
so we've我们已经 devised设计 an applicator敷贴.
81
213399
2084
03:47
And it's a very simple简单 device设备 --
82
215483
1487
03:48
you could call it a sophisticated复杂的 finger手指.
83
216970
2118
03:51
It's a spring-operated弹簧操作的 device设备.
84
219088
3040
03:54
What we do is when we apply应用
the NanopatchNanopatch to the skin皮肤 as so --
85
222128
3822
03:57
(Click
86
225950
1869
03:59
immediately立即 a few少数 things happen发生.
87
227819
2917
04:02
So firstly首先, the projections预测 on the NanopatchNanopatch
88
230736
4047
04:06
breach突破口 through通过 the tough强硬 outer layer
89
234783
1677
04:08
and the vaccine疫苗 is very quickly很快 released发布 --
90
236460
2046
04:10
within less than a minute分钟, in fact事实.
91
238506
2348
04:12
Then we can take the NanopatchNanopatch off
92
240854
2566
04:15
and discard丢弃 it.
93
243420
1938
04:17
And indeed确实 we can make
a reuse重用 of the applicator敷贴 itself本身.
94
245358
5886
04:23
So that gives you an idea理念 of the NanopatchNanopatch,
95
251244
2736
04:25
and immediately立即 you can see some key advantages优点.
96
253980
2576
04:28
We've我们已经 talked about it being存在 needle-free无针 --
97
256556
1940
04:30
these are projections预测 that you can't even see --
98
258496
2234
04:32
and, of course课程, we get around
99
260730
1711
04:34
the needle phobia恐怖症 issue问题 as well.
100
262441
3073
04:37
Now, if we take a step back and think about
101
265514
1931
04:39
these other two really important重要 advantages优点:
102
267445
3396
04:42
One is improved改善 immune免疫的
04:46
and the second第二 is getting得到 rid摆脱 of the cold chain.
104
274877
3594
04:50
So let's start开始 with the first one,
04:52
It takes a little while to get our heads around,
106
280843
2006
04:54
but I'll try to explain说明 it in simple简单 terms条款.
107
282849
3729
04:58
So I'll take a step back and explain说明 to you
108
286578
1850
05:00
how vaccines疫苗 work in a simple简单 way.
109
288428
3410
05:03
So vaccines疫苗 work by introducing引入 into our body身体
110
291838
2577
05:06
a thing called an antigen抗原
111
294415
2103
05:08
which哪一个 is a safe安全 form形成 of a germ病菌.
112
296518
3177
05:11
Now that safe安全 germ病菌, that antigen抗原,
113
299695
2020
05:13
tricks技巧 our body身体 into mounting安装 an immune免疫的 response响应,
114
301715
3505
05:17
learning学习 and remembering记忆
how to deal合同 with intruders入侵者.
115
305220
4068
05:21
When the real真实 intruder侵入者 comes along沿
116
309288
2368
05:23
the body身体 quickly很快 mounts坐骑 an immune免疫的 response响应
117
311656
1761
05:25
to deal合同 with that vaccine疫苗
118
313417
1636
05:27
and neutralizes中和 the infection感染.
119
315053
1900
05:28
So it does that well.
120
316953
1690
05:30
Now, the way it's doneDONE today今天
with the needle and syringe注射器,
121
318643
2337
05:32
most vaccines疫苗 are delivered交付 that way --
122
320980
2451
05:35
with this old technology技术 and the needle.
123
323431
1684
05:37
But it could be argued争论 that the needle
is holding保持 back our immune免疫的 responses回复;
124
325115
5149
05:42
it's missing失踪 our immune免疫的 sweet spot in the skin皮肤.
125
330264
3442
05:45
To describe描述 this idea理念,
126
333706
2996
05:48
we need to take a journey旅程 through通过 the skin皮肤,
127
336702
2489
05:51
starting开始 with one of those projections预测
128
339191
2624
05:53
and applying应用 the NanopatchNanopatch to the skin皮肤.
129
341815
2445
05:56
And we see this kind of data数据.
130
344260
2400
05:58
Now, this is real真实 data数据 --
131
346660
1809
06:00
that thing that we can see there is one projection投影
132
348469
2346
06:02
from the NanopatchNanopatch that's been applied应用的 to the skin皮肤
133
350815
2456
06:05
and those colors颜色 are different不同 layers.
134
353271
1925
06:07
Now, to give you an idea理念 of scale规模,
135
355196
1284
06:08
if the needle was shown显示 here, it would be too big.
136
356480
1940
06:10
It would be 10 times bigger
137
358420
1525
06:11
than the size尺寸 of that screen屏幕,
going 10 times deeper更深 as well.
138
359945
2922
06:14
It's off the grid entirely完全.
139
362867
2215
06:17
You can see immediately立即 that we
have those projections预测 in the skin皮肤.
140
365082
3193
06:20
That red layer is a tough强硬 outer layer of dead skin皮肤,

06:59
I found发现 that exciting扣人心弦. But so what?
156

In the world世界 of vaccines疫苗, what does that mean?
158
412478
2803
07:07
The world世界 of vaccines疫苗 is getting得到 better.
159
415281

07:09
It's getting得到 more systematic系统的.

It's a gambler's赌徒的 game游戏 even today今天.
165
426946
2702
07:21
So, we had to do that gamble.
166
429648

We obtained获得 an influenza流感 vaccine疫苗,

07:26
we applied应用的 it to our NanopatchesNanopatches

07:32
and this is in the live生活 animal动物.
171
440460
1828
07:34
We waited等待 a month,
172
442288
1851
07:36
and this is what we found发现 out.
173
444139

07:37
This is a data数据 slide滑动 showing展示 the immune免疫的 responses回复
174
445951
2325
07:40
that we've我们已经 generated产生 with a NanopatchNanopatch
175
448276
2323
07:42
compared相比 to the needle and syringe注射器 into muscle肌肉.
176
450599
3776
07:46
So on the horizontal axis we have
the dose剂量 shown显示 in nanograms纳克.
177
454375
3594
07:49
On the vertical垂直 axis we have
07:52
and that dashed虚线 line线 indicates指示
the protection保护 threshold.
179
460674
5515
07:58
If we're above以上 that line线 it's considered考虑 protective保护的;
180
466189
2453
如果在那个线上,则意味着是受保护的;
08:00
if we're below下面 that line线 it's not.
181
468642
2926
08:03
So the red line线 is mostly大多 below下面 that curve曲线
182
471568
2568
我们能看到,红色的线大多时候都在线下;
08:06
and indeed确实 there's only one point that
is achieved实现 with the needle that's protective保护的,
183
474136
3524
08:09
and that's with a high dose剂量 of 6,000 nanograms纳克.
184
477660
3209
08:12
But notice注意 immediately立即 the distinctly历历 different不同 curve曲线
185
480869
2533
08:15
that we achieve实现 with the blue蓝色 line线.
186
483402
3064
08:18
That's what's achieved实现 with the NanopatchNanopatch;
187
486466
1900
那是我们用纳米贴片得到的结果;
08:20
the delivered交付 dose剂量 of the NanopatchNanopatch is
188
488366
1737
08:25
That's a real真实 fresh新鲜 opportunity机会.
190
493464
1833
08:27
Suddenly突然 we have a brand new lever杠杆
191
495297
2378
08:29
in the world世界 of vaccines疫苗.
192
497675
1528
08:31
We can push it one way,
193
499203
1511
08:32
where we can take a vaccine疫苗
that works作品 but is too expensive昂贵
194
500714
2591
08:35
and can get protection保护
195
503305
1659
08:36
with a hundredth第一百 of the dose剂量
compared相比 to the needle.
196
504964
2824
08:39
That can take a vaccine疫苗 that's suddenly突然
10 dollars美元 down to 10 cents,
197
507788
4051
08:43
and that's particularly尤其 important重要
within the developing发展 world世界.
198
511839
3180
08:47
But there's another另一个 angle角度 to this as well --
199
515019
1867
08:48
you can take vaccines疫苗 that currently目前 don't work
200
516886
3052
08:51
and get them over that line线
201
519938
1196
08:53
and get them protective保护的.
202
521134
1958
08:55
And certainly当然 in the world世界 of vaccines疫苗
203
523092
2228
08:57
that can be important重要.
204
525320
1220
08:58
Let's consider考虑 the big three:
205
526540
1498
09:02
They're responsible主管 for about
7 million百万 deaths死亡 per year,
207
530735
2592
09:05
and there is no adequate充足 vaccination疫苗接种
method方法 for any of those.
208
533327
3207
09:08
So potentially可能, with this new lever杠杆
that we have with the NanopatchNanopatch,
209
536534
2532
09:11
we can help make that happen发生.
210
539066
1835
09:12
We can push that lever杠杆 to help get those
candidate候选人 vaccines疫苗 over the line线.
211
540901
4870
09:17
Now, of course课程, we've我们已经 worked工作 within my lab实验室
212
545771
1818
09:19
with many许多 other vaccines疫苗 that have attained实现
213
547589
1790
09:21
similar类似 responses回复 and similar类似 curves曲线 to this,
214
549379
2818
09:24
what we've我们已经 achieved实现 with influenza流感.
215
552197
3765
09:27
I'd like to now switch开关 to talk about
216
555962
2159
09:30
another另一个 key shortcoming缺点 of today's今天的 vaccines疫苗,
217
558121
3332
09:33
and that is the need to maintain保持 the cold chain.
218
561453
3420
09:36
As the name名称 suggests提示 -- the cold chain --
219
564873
2674
“冷链”,顾名思义
09:39
it's the requirements要求 of keeping保持
a vaccine疫苗 right from production生产
220
567547
2940
09:42
all the way through通过 to when the vaccine疫苗 is applied应用的,
221
570487
2676
09:45
to keep it refrigerated冷藏.
222
573163
3098
09:48
Now, that presents礼物 some logistical后勤 challenges挑战
223
576261
3246
09:51
but we have ways方法 to do it.
224
579507
2990
09:54
This is a slightly extreme极端 case案件 in point
225
582497
4536
09:59
but it helps帮助 illustrate说明 the logistical后勤 challenges挑战,
226
587033
2483
10:01
in particular特定 in resource-poor资源贫乏 settings设置,
227
589516
2390
10:03
of what's required需要 to get vaccines疫苗
228
591906
3286
10:07
refrigerated冷藏 and maintain保持 the cold chain.
229
595192
1718
10:08
If the vaccine疫苗 is too warm the vaccine疫苗 breaks休息 down,
230
596910
3814
10:12
but interestingly有趣 it can be too cold
231
600724
2207
10:14
and the vaccine疫苗 can break打破 down as well.
232
602931
2561
10:17
Now, the stakes赌注 are very high.
233
605492
3382
10:20
The WHO estimates估计 that within Africa非洲,
234
608874
2535
10:23
up to half the vaccines疫苗 used there
235
611409
2651
10:26
are considered考虑 to not be working加工 properly正确
236
614060
1853
10:27
because at some point the
cold chain has fallen堕落 over.
237
615913
2990
10:30
So it's a big problem问题, and it's tied
in with the needle and syringe注射器
238
618903
2566
10:33
because it's a liquid液体 form形成 vaccine疫苗, and
when it's liquid液体 it needs需求 the refrigeration冷藏.
239
621469
5233
10:38
A key attribute属性 of our NanopatchNanopatch
240
626702
2511
10:41
is that the vaccine疫苗 is dry,
241
629213
1973
10:43
and when it's dry it doesn't need refrigeration冷藏.
242
631186
2789
10:45
Within my lab实验室 we've我们已经 shown显示 that we can keep
243
633975
2412
10:48
the vaccine疫苗 stored存储 at 23 degrees Celsius摄氏
244
636387
2680
10:51
for more than a year without
any loss失利 in activity活动 at all.
245
639067
3716
10:54
That's an important重要 improvement起色.
246
642783
2387
10:57
(Applause
247
645170
6600
11:03
We're delighted欣喜的 about it as well.
248
651770
2463
11:06
And the thing about it is that
we have well and truly proven证明
249
654233
4296
11:10
the NanopatchNanopatch within the laboratory实验室 setting设置.
250
658529
2663
11:13
And as a scientist科学家, I love that and I love science科学.
251
661192
3973
作为一名科学家,我很爱这一点,我也热爱科学;
11:17
However然而, as an engineer工程师,
252
665165
2508
11:19
as a biomedical生物医药 engineer工程师
253
667673
1713
11:21
and also as a human人的 being存在,
254
669386
2203
11:23
I'm not going to be satisfied满意
255
671589
1231
11:24
until直到 we've我们已经 rolled热轧 this thing
out, taken采取 it out of the lab实验室
256
672820
2693
11:27
and got it to people in large numbers数字
257
675513
2431
11:29
and particularly尤其 the people that need it the most.
258
677944
3536
11:33
So we've我们已经 commenced开始 this particular特定 journey旅程,
259
681480
2702
11:36
and we've我们已经 commenced开始 this
journey旅程 in an unusual异常 way.
260
684182
2661
11:38
We've我们已经 started开始 with Papua巴布亚 New Guinea几内亚.
261
686843
2989
11:41
Now, Papua巴布亚 New Guinea几内亚 is an example
of a developing发展 world世界 country国家.
262
689832
5805
11:47
It's about the same相同 size尺寸 as France法国,
263
695637
3136
11:50
but it suffers患有 from many许多 of the key barriers障碍
264
698773
2835
11:53
existing现有 within the world世界 of today's今天的 vaccines疫苗.
265
701608
4068
11:57
There's the logistics
266
705676
1613
11:59
Within this country国家 there are only 800
refrigerators冰箱 to keep vaccines疫苗 chilled.
267
707289
4358
12:03
Many许多 of them are old, like this one in Port港口 Moresby莫尔斯比港,
many许多 of them are breaking破坏 down
268
711647
4293
12:07
and many许多 are not in the Highlands高地
where they are required需要.
269
715940
2916
12:10
That's a challenge
270
718856
1463
12:12
But also, Papua巴布亚 New Guinea几内亚 has the
world's世界 highest最高 incidence发生率 of HPVHPV,
271
720319
5183
12:21
Yet然而, that vaccine疫苗 is not available可得到 in large numbers数字
273
729733
2701
12:24
because it's too expensive昂贵.
274
732434
1914
12:26
So for those two reasons原因, with
the attributes属性 of the NanopatchNanopatch,
275
734348
2746
12:29
we've我们已经 got into the field领域 and
worked工作 with the NanopatchNanopatch,
276
737094
2615
12:31
and taken采取 it to Papua巴布亚 New Guinea几内亚
277
739709
2451
12:34
and we'll be following以下 that up shortly不久.
278
742160
3983
12:38
Now, doing this kind of work is not easy简单.
279
746143
3482
12:41
It's challenging

12:42
but there's nothing else其他 in
