ABOUT THE SPEAKER
Anthony Atala - Surgeon
Anthony Atala asks, "Can we grow organs instead of transplanting them?" His lab at the Wake Forest Institute for Regenerative Medicine is doing just that -- engineering over 30 tissues and whole organs.

Why you should listen

Anthony Atala is the director of the Wake Forest Institute for Regenerative Medicine, where his work focuses on growing and regenerating tissues and organs. His team engineered the first lab-grown organ to be implanted into a human -- a bladder -- and is developing experimental fabrication technology that can "print" human tissue on demand.

In 2007, Atala and a team of Harvard University researchers showed that stem cells can be harvested from the amniotic fluid of pregnant women. This and other breakthroughs in the development of smart bio-materials and tissue fabrication technology promises to revolutionize the practice of medicine.

There's actually a major health crisis today
0
0
3000
Sot ekziston një krizë e rëndë në shëndetësi
00:18
in terms of the shortage of organs.
1
3000
2000
në lidhje me pamjaftueshmërinë e organeve.
00:20
The fact is that we're living longer.
2
5000
2000
Po jetojmë më gjatë.
00:22
Medicine has done a much better job
3
7000
2000
Mjekësia e ka kryer punën më mirë
00:24
of making us live longer,
4
9000
2000
duke na e zgjatur jetën,
00:26
and the problem is, as we age,
5
11000
2000
e duke u plakur,
00:28
our organs tend to fail more,
6
13000
3000
organet tona fillojnë të degradohen,
00:31
and so currently
7
16000
2000
e në fakt sot
00:33
there are not enough organs to go around.
8
18000
2000
nuk ka organe të mjaftueshme për transplantim.
00:35
In fact, in the last 10 years,
9
20000
2000
Në fakt, në 10 vjetët e fundit,
00:37
the number of patients requiring an organ has doubled,
10
22000
3000
numri i pacientëve që presin për transplant është dyfishuar
00:40
while in the same time,
11
25000
2000
ndërkohë numri i transplanteve,
00:42
the actual number of transplants has barely gone up.
12
27000
3000
me zor se është shtuar.
00:45
So this is now a public health crisis.
13
30000
2000
Pra, kjo është një krizë e re e shëndetësisë.
00:47
So that's where this field comes in
14
32000
2000
Këtu pra, ndërhyn një fushë e cila
00:49
that we call the field of regenerative medicine.
15
34000
2000
quhet fusha e mjekësisë rigjenerative.
00:51
It really involves many different areas.
16
36000
2000
Në fakt ajo përfshin shumë zona të ndryshme.
00:53
You can use, actually, scaffolds,
17
38000
2000
Ju mund të përdorni, skelete,
00:55
biomaterials --
18
40000
2000
biomateriale--
00:57
they're like the piece of your blouse or your shirt --
19
42000
2000
ato janë si copa bluzash, pëlhure--
00:59
but specific materials you can actually implant in patients
20
44000
3000
por të përftuara për t'u implantuar në pacientë
01:02
and they will do well and help you regenerate.
21
47000
3000
dhe do t'ju ndihmojnë të ripërtërihen.
01:05
Or we can use cells alone,
22
50000
3000
Ose mund të përdorim vetëm qeliza,
01:08
either your very own cells
23
53000
2000
ose vetë qelizat tuaja
01:10
or different stem cell populations.
24
55000
2000
ose popullata të ndryshme qelizash burimore (staminale)
01:12
Or we can use both.
25
57000
2000
Ose edhe të dyja.
01:14
We can use, actually, biomaterials
26
59000
2000
Ne mund të përdorim, biomateriale
01:16
and the cells together.
27
61000
2000
dhe qeliza sëbashku.
01:18
And that's where the field is today.
28
63000
3000
Dhe në këtë gjendje është fusha sot.
01:21
But it's actually not a new field.
29
66000
2000
Por në fakt s'është fushë e re.
01:23
Interestingly, this is a book
30
68000
2000
Çuditërisht, ky është një libër
01:25
that was published back in 1938.
31
70000
3000
që është publikuar në vitin 1938.
01:28
It's titled "The Culture of Organs."
32
73000
2000
Quhet "Kultura e Organeve".
01:30
The first author, Alexis Carrel, a Nobel Prize winner.
33
75000
3000
Autori i parë, Alexis Carrel, fitues i çmimit Nobel.
01:33
He actually devised some of the same technologies
34
78000
2000
Ai shpiku disa nga teknologjitë
01:35
used today for suturing blood vessels,
35
80000
2000
që përdoren sot për qepjen e enëve të gjakut,
01:37
and some of the blood vessel grafts we use today
36
82000
3000
e disa nga graftet (transplantet) që ne përdorim
01:40
were actually designed by Alexis.
37
85000
3000
kanë qenë të projektuara nga Alexis.
01:43
But I want you to note his co-author:
38
88000
3000
Ju duhet të vini re bashkëautorin e vet:
01:46
Charles Lindbergh.
39
91000
2000
Charles Lindbergh.
01:48
That's the same Charles Lindbergh
40
93000
3000
I njejti Charles Lindbergh
01:51
who actually spent the rest of his life
41
96000
2000
i cili shpenzoi tërë jetën e tij
01:53
working with Alexis
42
98000
2000
duke punuar me Alexis'in
01:55
at the Rockefeller Institute in New York
43
100000
2000
në Institutin Rockefeller në New York
01:57
in the area of the culture of organs.
44
102000
2000
në fushën e kulturës së organeve.
01:59
So if the field's been around for so long,
45
104000
2000
Por, n.q.s këto gjera ka kohë që dihen,
02:01
why so few clinical advances?
46
106000
2000
pse kaq pak përparime në klinikë?
02:03
And that really has to do to many different challenges.
47
108000
3000
Kjo ka të bëjë me numrin e madh të veshtirësive.
02:06
But if I were to point to three challenges,
48
111000
2000
por, tri problemet kryesore janë,
02:08
the first one is actually the design of materials
49
113000
2000
Një-- dizajni i materialeve që mund të
02:10
that could go in your body
50
115000
2000
vendosen brenda trupit
02:12
and do well over time.
51
117000
2000
dhe nuk krijojnë probleme me kalimin e kohës
02:14
And many advances now,
52
119000
2000
Shumë përparime tani,
02:16
we can do that fairly readily.
53
121000
2000
ne jemi në gjendje t'i kryejmë.
02:18
The second challenge was cells.
54
123000
2000
Dy-- qelizat.
02:20
We could not get enough of your cells to grow outside of your body.
55
125000
3000
Nuk kishim mundësinë që të rrisnim një numër të mjaftueshëm qelizash
02:23
Over the last 20 years, we've basically tackled that.
56
128000
3000
Gjatë 20 vjetëve të fundit, e kemi zgjidhur këtë.
02:26
Many scientists can now grow many different types of cells.
57
131000
2000
Shkencëtarët janë në gjendje të rrisin shumë lloje qelizash.
02:28
Plus we have stem cells.
58
133000
2000
Kemi gjithashtu edhe qeliza staminale.
02:30
But even now, 2011,
59
135000
3000
Por edhe sot, në 2011,
02:33
there's still certain cells that we just can't grow from the patient.
60
138000
4000
Ka disa lloje qelizash të cilat ende nuk mundemi t'i rrisim nga pacienti.
02:37
Liver cells, nerve cells, pancreatic cells --
61
142000
3000
Qelizat e mëlçisë, qelizat nervore dhe qelizat e pankreasit--
02:40
we still can't grow them even today.
62
145000
3000
Nuk jemi në gjendje t'i rrisim as sot.
02:43
And the third challenge is vascularity,
63
148000
2000
Së treti, problemi i vaskularitetit,
02:45
the actual supply of blood
64
150000
3000
furnizimit me gjak
02:48
to allow those organs or tissues to survive
65
153000
3000
për t'u mundësuar atyre organeve mbijetesën
02:51
once we regenerate them.
66
156000
2000
sapo i kemi rigjeneruar.
02:53
So we can actually use biomaterials now.
67
158000
2000
Pra, sot ne mund të përdorim biomateriale.
02:55
This is actually a biomaterial.
68
160000
2000
Ky është në fakt biomaterial.
02:57
We can weave them, knit them, or we can make them like you see here.
69
162000
3000
Ne mund t'i tjerrim, t'i qendisim, ose t'i ndreqim kështu.
03:00
This is actually like a cotton candy machine.
70
165000
3000
kjo është si makina e sheqerit- pambuk.
03:03
You saw the spray going in.
71
168000
2000
Ja si po e spërkasin
03:05
That was like the fibers of the cotton candy
72
170000
2000
Përnjimend si fijet e sheqerit- pambuk
03:07
creating this structure, this tubularized structure,
73
172000
2000
këto struktura, struktura tubulare,
03:09
which is a biomaterial
74
174000
2000
Si biomaterial
03:11
that we can then use
75
176000
2000
që ne mund të përdorim
03:13
to help your body regenerate
76
178000
2000
për të ndihmuar ripërtëritjen
03:15
using your very own cells to do so.
77
180000
3000
duke përdorur vet qelizat tuaja.
03:18
And that's exactly what we did here.
78
183000
2000
Dhe pikërisht këtë e bëmë këtu.
03:20
This is actually a patient
79
185000
2000
Ky është një pacient
03:22
who [was] presented with a deceased organ,
80
187000
2000
që u paraqit me një organ jo vital
03:24
and we then created one of these smart biomaterials,
81
189000
3000
dhe ne krijuam një prej këtyre biomaterialeve inteligjente
03:27
and then we then used that smart biomaterial
82
192000
2000
dhe i përdorëm ato më tej
03:29
to replace and repair
83
194000
3000
për të zëvendësuar dhe riparuar
03:32
that patient's structure.
84
197000
2000
strukturat e pacientit.
03:34
What we did was we actually
85
199000
2000
Ajo që ne bëmë është që ne
03:36
used the biomaterial as a bridge
86
201000
2000
përdorëm biomaterialet si një urë
03:38
so that the cells in the organ
87
203000
2000
që qelizat në organ të
03:40
could walk on that bridge, if you will,
88
205000
2000
kalojnë urën, dhe të
03:42
and help to bridge the gap
89
207000
2000
plotësojnë boshllëqet
03:44
to regenerate that tissue.
90
209000
2000
e të ripërtërijnë indet.
03:46
And you see that patient now six months after
91
211000
2000
Dhe e vëreni tani pacientin mbas 6 muajsh
03:48
with an X-ray showing you the regenerated tissue,
92
213000
3000
një grafi ju tregon indin e ripërtërirë,
03:51
which is fully regenerated
93
216000
2000
plotësisht të ripërtërirë
03:53
when you analyze it under the microscope.
94
218000
3000
kur e analizoni nën mikroskop.
03:56
We can also use cells alone.
95
221000
2000
Ne mund të përdorim qoftë edhe vetëm qeliza.
03:58
These are actually cells that we obtained.
96
223000
3000
Këto janë qelizat që përftuam.
04:01
These are stem cells that we create from specific sources,
97
226000
3000
Qeliza staminale që ne i krijojmë nga burime të caktuara,
04:04
and we can drive them to become heart cells,
98
229000
2000
i detyrojmë që të diferencohen si qeliza të zemrës,
04:06
and they start beating in culture.
99
231000
2000
dhe ato fillojnë të rrahin në kulturë.
04:08
So they know what to do.
100
233000
2000
Pra, ato e njohin detyrën e tyre.
04:10
The cells genetically know what to do,
101
235000
2000
gjenetikisht ato janë të predispozuara,
04:12
and they start beating together.
102
237000
2000
të rrahin sëbashku.
04:14
Now today, many clinical trials
103
239000
2000
Sot e kësaj dite, shumë studime klinike
04:16
are using different kinds of stem cells
104
241000
2000
po përdorin lloje të ndryshme qelizash burimore
04:18
for heart disease.
105
243000
2000
për sëmundjet e zemrës.
04:20
So that's actually now in patients.
106
245000
3000
Kjo aktualisht ndodhet në pacientë.
04:23
Or if we're going to use larger structures
107
248000
2000
ose nëse do të përdorim struktura më të mëdha
04:25
to replace larger structures,
108
250000
2000
të zëvendësojmë struktura më të mëdha,
04:27
we can then use the patient's own cells,
109
252000
2000
ne mund të përdorim qelizat e vet pacientit,
04:29
or some cell population,
110
254000
2000
ose ndonjë popullatë qelizash,
04:31
and the biomaterials,
111
256000
2000
e biomaterialet,
04:33
the scaffolds, together.
112
258000
2000
strukturat, sëbashku.
04:35
So the concept here:
113
260000
2000
Pra, koncepti është ky:
04:37
so if you do have a deceased or injured organ,
114
262000
3000
nga organi juaj i dëmtuar apo jashtë përdorimi,
04:40
we take a very small piece of that tissue,
115
265000
2000
marrim një pjesë të vogël nga indi
04:42
less than half the size of a postage stamp.
116
267000
3000
më pak se ½ e një pulle poste.
04:45
We then tease the cells apart,
117
270000
3000
I ndajmë qelizat nga njera- tjetra,
04:48
we grow the cells outside the body.
118
273000
2000
i rrisim ato jashtë trupit.
04:50
We then take a scaffold, a biomaterial --
119
275000
3000
Dhe marrim një skelet, biomaterial--
04:53
again, looks very much like a piece of your blouse or your shirt --
120
278000
3000
përsëri, duket pikërisht si copë pëlhurë nga bluza juaj--
04:56
we then shape that material,
121
281000
2000
i japim formë,
04:58
and we then use those cells to coat that material
122
283000
3000
dhe përdorim qeliza që ti mbulojmë
05:01
one layer at a time --
123
286000
2000
shtresë mbas shtrese--
05:03
very much like baking a layer cake, if you will.
124
288000
3000
tamam si tortë me kate.
05:06
We then place it in an oven-like device,
125
291000
2000
I vendosim në një si furrë
05:08
and we're able to create that structure
126
293000
2000
dhe krijojmë strukturën
05:10
and bring it out.
127
295000
2000
dhe e nxjerrim atë.
05:12
This is actually a heart valve
128
297000
2000
Kjo është kllapë zemre
05:14
that we've engineered,
129
299000
2000
që kemi prodhuar,
05:16
and you can see here, we have the structure of the heart valve
130
301000
3000
dhe ju mund ta shihni këtu, strukturën
05:19
and we've seeded that with cells,
131
304000
3000
që po mbjellim me qeliza,
05:22
and then we exercise it.
132
307000
2000
e i ushtrojmë ato.
05:24
So you see the leaflets opening and closing --
133
309000
2000
ja si hapen e mbyllen fletët--
05:26
of this heart valve
134
311000
2000
e kësaj valvule
05:28
that's currently being used experimentally
135
313000
4000
e cila është aktualisht eksperimentale
05:32
to try to get it to further studies.
136
317000
3000
dhe që po përpiqemi të përmirësojmë.
05:35
Another technology
137
320000
2000
Një teknologji tjetër
05:37
that we have used in patients
138
322000
2000
që kemi përdorur tek pacientët
05:39
actually involves bladders.
139
324000
2000
ka të bëjë me fshikëzat.
05:41
We actually take a very small piece of the bladder from the patient --
140
326000
3000
Marrim një copë të vockël nga fshikëza e urinës të pacientit--
05:44
less than half the size of a postage stamp.
141
329000
3000
më pak se ½ e një pulle poste.
05:47
We then grow the cells outside the body,
142
332000
2000
Dhe i rrisim qelizat jashtë trupit,
05:49
take the scaffold, coat the scaffold with the cells --
143
334000
2000
marrim skeletin, e mbulojmë atë me qeliza--
05:51
the patient's own cells, two different cell types.
144
336000
3000
qelizat e vet pacientit, dy lloje të ndryshme qelizash.
05:54
We then put it in this oven-like device.
145
339000
3000
dhe i vendosim në "furrë".
05:57
It has the same conditions as the human body --
146
342000
2000
Me të njëjtat kushte si në brendësi të trupit--
05:59
37 degrees centigrade, 95 percent oxygen.
147
344000
3000
37 gradë temperaturë, 95 % oksigjen.
06:02
A few weeks later, you have your engineered organ
148
347000
3000
Disa javë më vonë, përftojmë organe që ne mund
06:05
that we're able to implant back into the patient.
149
350000
3000
t'i implantojmë përsëri brenda pacientit.
06:08
For these specific patients, we actually just suture these materials.
150
353000
3000
Ne i qepim këto materiale, për këta pacientë.
06:11
We use three-dimensional imagining analysis,
151
356000
3000
Përdorim analizë tre-dimensionale të imazheve,
06:14
but we actually created these biomaterials by hand.
152
359000
3000
por i krijojmë me dorë.
06:18
But we now have better ways
153
363000
2000
Por sot kemi metoda më të mira
06:20
to create these structures with the cells.
154
365000
2000
për të krijuar këto struktura me qeliza.
06:22
We use now some type of technologies,
155
367000
4000
Sot përdorim disa teknologji të reja,
06:26
where for solid organs, for example,
156
371000
2000
Ku për disa organe solide, si p.sh.
06:28
like the liver,
157
373000
2000
mëlçinë,
06:30
what we do is we take discard livers.
158
375000
3000
ne marrim organe të skarcuara.
06:33
As you know, a lot of organs are actually discarded, not used.
159
378000
3000
Siç e dini, shumë organe skarcohen, nuk përdoren.
06:36
So we can take these liver structures,
160
381000
2000
Pra, ne i marrim këto struktura të mëlçisë,
06:38
which are not going to be used,
161
383000
2000
të cilat do të përdoren,
06:40
and we then put them in a washing machine-like structure
162
385000
4000
e pastaj i vendosim në një si makinë larëse
06:44
that will allow the cells to be washed away.
163
389000
2000
që i shpëlan qelizat.
06:46
Two weeks later,
164
391000
2000
Dy javë më vonë,
06:48
you have something that looks like a liver.
165
393000
2000
përftojmë diçka që i ngjan mëlçisë
06:50
You can hold it like a liver,
166
395000
2000
që mbahet në dorë si mëlçi,
06:52
but it has no cells; it's just a skeleton of the liver.
167
397000
3000
por nuk ka asnjë qelizaë; është thjesht skeleti i mëlçisë.
06:55
And we then can re-perfuse the liver with cells,
168
400000
4000
Pastaj e riperfuzojmë mëlçinë me qeliza,
06:59
preserving the blood vessel tree.
169
404000
2000
duke e ruajtur trungun vaskular.
07:01
So we actually perfuse first the blood vessel tree
170
406000
3000
Ne në fakt e riperfuzojmë në fillim trungun vaskular me qelizat e enëve
07:04
with the patient's own blood vessel cells,
171
409000
2000
të gjakut, qelizat e vet pacientit,
07:06
and we then infiltrate the parenchyma with the liver cells.
172
411000
3000
e pastaj e infiltrojmë parenkimën me qeliza të mëlçisë.
07:09
And we now have been able just to show
173
414000
2000
ja si ja dolëm të vërtetojmë
07:11
the creation of human liver tissue
174
416000
2000
krijimin e indit të mëlçisë njerëzore.
07:13
just this past month
175
418000
2000
gjatë muajit të fundit
07:15
using this technology.
176
420000
3000
duke përdorur këtë teknologji.
07:19
Another technology that we've used
177
424000
2000
Një teknologji tjetër e përdorur është
07:21
is actually that of printing.
178
426000
2000
printimi.
07:23
This is actually a desktop inkjet printer,
179
428000
3000
Ky është një printer inkxhet,
07:26
but instead of using ink,
180
431000
2000
por në vend të bojës,
07:28
we're using cells.
181
433000
2000
ne përdorim qeliza.
07:30
And you can actually see here the printhead
182
435000
2000
Ja ku po e shikoni kokën e printerit
07:32
going through and printing this structure,
183
437000
2000
që kalon përmes strukturës
07:34
and it takes about 40 minutes to print this structure.
184
439000
2000
dhe i duhen rreth 40 minuta për ta printuar atë.
07:36
And there's a 3D elevator
185
441000
2000
Ka një ashensor në 3D
07:38
that then actually goes down one layer at a time
186
443000
2000
që kalon përmes secilës shtresë
07:40
each time the printhead goes through.
187
445000
2000
gjatë printimit.
07:42
And then finally you're able to get that structure out.
188
447000
3000
Dhe në fund e nxjerrim strukturën.
07:45
You can pop that structure out of the printer and implant it.
189
450000
3000
E nxjerrim nga printeri dhe mund ta implantojmë.
07:48
And this is actually a piece of bone
190
453000
2000
Ja këtu kemi një copë kockë
07:50
that I'm going to show you in this slide
191
455000
3000
që po ju tregoj në diapozitivë
07:53
that was actually created with this desktop printer
192
458000
3000
e krijuar pikërisht me këtë printerin
07:56
and implanted as you see here.
193
461000
3000
dhe u implantua këtu.
07:59
That was all new bone that was implanted
194
464000
2000
Kockë krejtësisht e re e implantuar
08:01
using these techniques.
195
466000
3000
duke përdorur këto struktura.
08:04
Another more advanced technology we're looking at right now,
196
469000
4000
Një teknologji tjetër që po studiojmë tani,
08:08
our next generation of technologies,
197
473000
2000
brezi i ri teknologjik,
08:10
are more sophisticated printers.
198
475000
2000
përfaqësohet nga printera më të sofistikuar.
08:12
This particular printer we're designing now
199
477000
3000
ky printeri i veçantë që po dizenjojmë tani
08:15
is actually one where we print right on the patient.
200
480000
3000
është model që printon direkt mbi pacient.
08:18
So what you see here --
201
483000
2000
ja ku po e shikoni--
08:20
I know it sounds funny,
202
485000
2000
tingullon qesharake,
08:22
but that's the way it works.
203
487000
2000
por tamam kështu funksionon.
08:24
Because in reality, what you want to do
204
489000
3000
sepse në realitet, ju e doni pacientin
08:27
is you actually want to have the patient on the bed with the wound,
205
492000
3000
të shtrirë mbi tavolinë me plagë të ekspozuar,
08:30
and you have a scanner,
206
495000
2000
e keni një skaner,
08:32
basically like a flatbed scanner.
207
497000
2000
si skanerat e sheshtë.
08:34
That's what you see here on the right side.
208
499000
2000
Këtë po e shikoni nga e djathta.
08:36
You see a scanner technology
209
501000
2000
teknologjia e skanerit
08:38
that first scans the wound on the patient
210
503000
3000
që në fillim skanon plagën e pacientit
08:41
and then it comes back with the printheads
211
506000
3000
dhe më vonë kalon me kokën e printerit
08:44
actually printing the layers that you require
212
509000
3000
dhe printon shtresën e kërkuar të indit
08:47
on the patients themselves.
213
512000
2000
mbi vet pacientin.
08:49
This is how it actually works.
214
514000
2000
Ja dhe parimi i punës.
08:51
Here's the scanner going through,
215
516000
2000
ja si po kalon skaneri,
08:53
scanning the wound.
216
518000
2000
skanon plagën.
08:55
Once it's scanned,
217
520000
2000
Sa të skanohet,
08:57
it sends information in the correct layers of cells
218
522000
2000
merr informacione të sakta mbi shtresat e qelizave
08:59
where they need to be.
219
524000
2000
që na nevojiten.
09:01
And now you're going to see here
220
526000
2000
Dhe po i shikoni këtu
09:03
a demo of this actually being done
221
528000
2000
një demostrim të punës
09:05
in a representative wound.
222
530000
3000
në plagë maket.
09:08
And we actually do this with a gel so that you can lift the gel material.
223
533000
3000
E ne e realizojmë këtë nëpërmjet një xheli, që mund të manipulohet.
09:11
So once those cells are on the patient
224
536000
2000
Dhe sa të shtrohen,
09:13
they will stick where they need to be.
225
538000
2000
qelizat ngjisin në vendin e duhur.
09:15
And this is actually new technology
226
540000
2000
Dhe kjo akoma është
09:17
still under development.
227
542000
3000
në etap zhvillimi.
09:20
We're also working on more sophisticated printers.
228
545000
3000
Ne gjithashtu po punojmë mbi printera më të sofistikuar.
09:23
Because in reality, our biggest challenge
229
548000
2000
Sepse, përnjimend sfida jonë më e madhe
09:25
are the solid organs.
230
550000
2000
janë organet solide.
09:27
I don't know if you realize this,
231
552000
2000
Unë nuk e di nëse e konceptoni,
09:29
but 90 percent of the patients on the transplant list
232
554000
4000
por 90% e pacientëve në listë
09:33
are actually waiting for a kidney.
233
558000
3000
presin për veshkë.
09:36
Patients are dying every day
234
561000
2000
Po vdesin njerëz çdo ditë,
09:38
because we don't have enough of those organs to go around.
235
563000
3000
sepse ne nuk kemi organe të mjaftueshme për të gjithë.
09:41
So this is more challenging --
236
566000
2000
Pra, kjo është një sfidë e rëndë--
09:43
large organ, vascular,
237
568000
2000
organ i madh, i vaskularizuar,
09:45
a lot of blood vessel supply,
238
570000
2000
furnizim i madh me gjak,
09:47
a lot of cells present.
239
572000
2000
dendësi e madhe qelizash.
09:49
So the strategy here is --
240
574000
2000
Pra, strategjia është e tillë--
09:51
this is actually a CT scan, an X-ray --
241
576000
2000
kemi një TK, skaner, grafi--
09:53
and we go layer by layer,
242
578000
2000
dhe kalojmë shtresë për shtresë,
09:55
using computerized morphometric imaging analysis
243
580000
2000
duke përdorur analizën kompjuterike morfometrike të imazheve
09:57
and 3D reconstruction
244
582000
2000
dhe rindërtim 3D
09:59
to get right down to those patient's own kidneys.
245
584000
3000
që të përftojmë veshkën e vet pacientit.
10:02
We then are able to actually image those,
246
587000
3000
Sapo i kemi imazhet e tyre,
10:05
do 360 degree rotation
247
590000
2000
e kthejmë me 360 gradë
10:07
to analyze the kidney
248
592000
2000
për të analizuar veshkën
10:09
in its full volumetric characteristics,
249
594000
4000
në të gjitha karakteristikat volumetrike,
10:13
and we then are able
250
598000
2000
e jemi të aftë
10:15
to actually take this information
251
600000
2000
ta marrim këtë informacion
10:17
and then scan this
252
602000
2000
dhe ta përdorim
10:19
in a printing computerized form.
253
604000
2000
për të printuar me kompjuter.
10:21
So we go layer by layer through the organ,
254
606000
3000
Shtresë mbas shtrese,
10:24
analyzing each layer as we go through the organ,
255
609000
3000
duke analizuar secilën shtresë,
10:27
and we then are able to send that information, as you see here,
256
612000
4000
dhe e dërgojmë informacionin, siç e shihni këtu,
10:31
through the computer
257
616000
2000
përmes kompjuterit
10:33
and actually design the organ
258
618000
2000
dhe e dizenjojmë organin
10:35
for the patient.
259
620000
2000
për pacientin.
10:37
This actually shows the actual printer.
260
622000
3000
Kjo tregon printerin
10:40
And this actually shows that printing.
261
625000
2000
dhe produktin e tij
10:42
In fact, we actually have the printer right here.
262
627000
4000
Në fakt, ne e kemi një printer të pranishëm këtu.
10:46
So while we've been talking today,
263
631000
3000
Dhe gjatë kohës që kemi folur sot,
10:49
you can actually see the printer
264
634000
3000
ju mund ta shikoni vet printerin
10:52
back here in the back stage.
265
637000
3000
këtu në prapaskenë.
10:55
That's actually the actual printer right now,
266
640000
2000
Ky është vet printeri që tani
10:57
and that's been printing this kidney structure
267
642000
2000
po printon strukturën e veshkës
10:59
that you see here.
268
644000
2000
që po shikoni.
11:01
It takes about seven hours to print a kidney,
269
646000
2000
Atij i duhen rreth 7 orë që ta përfundojë,
11:03
so this is about three hours into it now.
270
648000
3000
dhe ky ka 3 orë që po punon.
11:06
And Dr. Kang's going to walk onstage right now,
271
651000
3000
Dhe Dr. Kangu po hipën tash mbi skenë
11:09
and we're actually going to show you one of these kidneys
272
654000
3000
dhe do t'ju tregojmë një prej këtyre veshkave
11:12
that we printed a little bit earlier today.
273
657000
3000
që printuam pak më herët sot.
11:18
Put a pair of gloves here.
274
663000
2000
Po vendos një çift dorashka.
11:26
Thank you.
275
671000
2000
Faleminderit!
11:28
Go backwards.
276
673000
2000
Ec mbrapa.
11:36
So, these gloves are a little bit small on me, but here it is.
277
681000
3000
Këto më rrijnë pak të vogla, po ja ku është!
11:39
You can actually see that kidney
278
684000
2000
Ja veshka që printuam sot.
11:41
as it was printed earlier today.
279
686000
2000
pak më herët.
11:43
(Applause)
280
688000
16000
(Duartrokitje)
11:59
Has a little bit of consistency to it.
281
704000
3000
ësht disi konsistente.
12:03
This is Dr. Kang who's been working with us on this project,
282
708000
3000
Ky është Dr. Kangu që ka punuar mbi këtë projekt,
12:06
and part of our team.
283
711000
2000
pjesëtar i ekipit tonë.
12:08
Thank you, Dr. Kang. I appreciate it.
284
713000
3000
Faleminderit, Dr. Kang. E vlerësoj.
12:11
(Applause)
285
716000
5000
(Duartrokitje)
12:16
So this is actually a new generation.
286
721000
2000
Kjo pra është gjenerata e re.
12:18
This is actually the printer that you see here onstage.
287
723000
2000
Printerin po e shikoni
12:20
And this is actually a new technology we're working on now.
288
725000
3000
dhe kjo është teknologjia që po zhvillojmë sot.
12:24
In reality, we now have a long history of doing this.
289
729000
4000
Në fakt, kemi shumë kohë që po merremi me këtë.
12:28
I'm going to share with you a clip
290
733000
2000
Dua të ndaj me ju një klip
12:30
in terms of technology we have had in patients now for a while.
291
735000
3000
të teknologjive me të cilat kemi kohë që po punojmë me pacientë.
12:33
And this is actually a very brief clip --
292
738000
2000
Një klip mjaft i shkurtër--
12:35
only about 30 seconds --
293
740000
2000
vetëm rreth 30 sekonda--
12:37
of a patient who actually received an organ.
294
742000
3000
të një pacienti që përftoi organ.
12:40
(Video) Luke Massella: I was really sick. I could barely get out of bed.
295
745000
2000
(Video) Luke Massella: unë isha shumë i sëmurë. Mezi ngrihesha nga krevati.
12:42
I was missing school. It was pretty much miserable.
296
747000
3000
Bëja mungesa në shkollë. Gjendja ishte e rëndë.
12:45
I couldn't go out
297
750000
2000
Nuk mund të dilja jashtë
12:47
and play basketball at recess
298
752000
2000
dhe të luaja basketboll
12:49
without feeling like I was going to pass out
299
754000
2000
pa pasur ndjenjën se do më binte të fikët
12:51
when I got back inside.
300
756000
2000
sa të kthehesha brenda.
12:53
I felt so sick.
301
758000
3000
Ndihesha i sëmurë.
12:56
I was facing basically a lifetime of dialysis,
302
761000
3000
Përpara vetes kisha një jetë me dializë,
12:59
and I don't even like to think about what my life would be like
303
764000
2000
as që doja ta përfytyroja një jetë të tillë
13:01
if I was on that.
304
766000
2000
kaq keq.
13:03
So after the surgery,
305
768000
2000
Por, mbas operacionit,
13:05
life got a lot better for me.
306
770000
2000
jeta mu përmirësua ndjeshëm.
13:07
I was able to do more things.
307
772000
2000
Unë mund të bëja shumë gjera.
13:09
I was able to wrestle in high school.
308
774000
2000
Mund të bëja mundje në gjimnaz.
13:11
I became the captain of the team, and that was great.
309
776000
3000
u bëra kapiten skuadre, ishte e mrekullueshme.
13:14
I was able to be a normal kid with my friends.
310
779000
3000
Kisha mundësinë të isha fëmijë normal me shokët.
13:17
And because they used my own cells to build this bladder,
311
782000
3000
Dhe duke qenë se qelizat ishin të mijat,
13:20
it's going to be with me.
312
785000
2000
do të më ngelet.
13:22
I've got it for life, so I'm all set.
313
787000
3000
Për tërë jetën, pra gati jam.
13:25
(Applause)
314
790000
38000
(Duartrokitje)
14:03
Juan Enriquez: These experiments sometimes work,
315
828000
2000
Juan Enriquez: Këto eksperimente nganjëherë janë të
14:05
and it's very cool when they do.
316
830000
2000
suksesshme dhe është tullë kur kjo ndodh.
14:07
Luke, come up please.
317
832000
2000
Luke, afrohu të lutem.
14:14
(Applause)
318
839000
15000
(Duartrokitje)
14:29
So Luke, before last night,
319
854000
2000
Pra, Luke, që parmbrëmë,
14:31
when's the last time you saw Tony?
320
856000
2000
kur u takove për herë të fundit me Tonin?
14:33
LM: Ten years ago, when I had my surgery --
321
858000
3000
LM: Para 10 vjetësh, kur u operova--
14:36
and it's really great to see him.
322
861000
2000
dhe është kënaqësi ta shoh përsëri.
14:38
(Laughter)
323
863000
2000
(Hilaritet)
14:40
(Applause)
324
865000
6000
(Duartrokitje)
14:46
JE: And tell us a little bit about what you're doing.
325
871000
2000
JE: Tregona pak se ç'je duke bërë.
14:48
LM: Well right now I'm in college at the University of Connecticut.
326
873000
3000
LM: Po ja, po bëj shkollën e lartë në Universitetin e Konektikatit.
14:51
I'm a sophomore and studying communications, TV and mass media,
327
876000
4000
jam në vitin e dytë dhe po studioj komunikacione, TV dhe mas media,
14:55
and basically trying to live life like a normal kid,
328
880000
3000
e po përpiqem të jetoj jetën normale të çdo djali,
14:58
which I always wanted growing up.
329
883000
2000
siç kam dashur gjithmonë të bëja kur të rritesha.
15:00
But it was hard to do that when I was born with spina bifida
330
885000
2000
E vështirë qe, se u linda me spina bifida,
15:02
and my kidneys and bladder weren't working.
331
887000
3000
e veshkat dhe fshikëza e urinës nuk punonin.
15:05
I went through about 16 surgeries,
332
890000
2000
Kalova përmes 16 operacionesh,
15:07
and it seemed impossible to do that
333
892000
2000
dukej e pamundur t'ia arrija kësaj
15:09
when I was in kidney failure when I was 10.
334
894000
3000
kur isha 10 vjeç e pësova pamjaftueshmëri të veshkave
15:12
And this surgery came along
335
897000
2000
dhe u shfaq kjo mundësi
15:14
and basically made me who I am today
336
899000
2000
që më bëri atë që jam sot.
15:16
and saved my life.
337
901000
2000
dhe më shpëtoi jetën.
15:18
(Applause)
338
903000
11000
(Duartrokitje)
15:29
JE: And Tony's done hundreds of these?
339
914000
3000
JE: Dhe Toni ka bërë me qindra të tilla?
15:32
LM: What I know from, he's working really hard in his lab
340
917000
3000
LM: Me sa e di unë, ai po punon shumë në laboratorin e tij
15:35
and coming up with crazy stuff.
341
920000
2000
dhe po bën këto gjera të çuditshme.
15:37
I know I was one of the first 10 people to have this surgery.
342
922000
3000
Unë kam qenë ndër 10 të parët që kam kaluar këtë operacion.
15:40
And when I was 10, I didn't realize how amazing it was.
343
925000
3000
Dhe kur isha 10 vjeç, nuk e merrja dot me mend sa
15:43
I was a little kid, and I was like,
344
928000
2000
e fortë ishte. Kur isha fëmijë thashë:
15:45
"Yeah. I'll have that. I'll have that surgery."
345
930000
2000
"Posi! Do ta bëj! Do ta bëj atë operacion!"
15:47
(Laughter)
346
932000
2000
(Hilaritet)
15:49
All I wanted to do was to get better,
347
934000
2000
Unë veç shpresoja të bëhesha më mirë,
15:51
and I didn't realize how amazing it really was until now that I'm older
348
936000
3000
s'ma priste mendja sa mirë do të bëhesha kur të rritesha
15:54
and I see the amazing things that he's doing.
349
939000
3000
e tani po shoh gjerat e mrekullueshme që ai po bën.
15:58
JE: When you got this call out of the blue --
350
943000
3000
JE: Kur të thirrëm ashtu, si pa paralajmërim--
16:01
Tony's really shy,
351
946000
2000
se Toni është pakëz i turpshëm
16:03
and it took a lot of convincing
352
948000
2000
dhe mu desh shumë për t'ia mbushur mendjen
16:05
to get somebody as modest as Tony
353
950000
2000
njërit kaq modest si Tonit
16:07
to allow us to bring Luke.
354
952000
3000
të më lejonte të sillja Luke'un.
16:10
So Luke, you go to your communications professors --
355
955000
2000
Pra Luke, ti vajte tek profesorët e tu të komunikacioneve--
16:12
you're majoring in communications --
356
957000
2000
e ke masterin në komunikacione--
16:14
and you ask them for permission to come to TED,
357
959000
2000
dhe i kërkove leje që të vish në TED,
16:16
which might have a little bit to do with communications,
358
961000
2000
që edhe mund të ketë pak lidhje me komunikimin,
16:18
and what was their reaction?
359
963000
3000
e cili qe reaksioni?
16:21
LM: Most of my professors were all for it,
360
966000
2000
LM: Shumica e profesorëve ishin me mua,
16:23
and they said, "Bring pictures
361
968000
3000
e thanë: "Na sill disa fotografi
16:26
and show me the clips online," and "I'm happy for you."
362
971000
3000
e na i trego klipet onlajn", e "më bëhet qejfi për ty"
16:29
There were a couple that were a little stubborn,
363
974000
2000
Ka pasë nja dy që ishin kokëfortë,
16:31
but I had to talk to them.
364
976000
2000
por ia mbusha mendjen.
16:33
I pulled them aside.
365
978000
3000
I mora mënjanë.
16:36
JE: Well, it's an honor and a privilege to meet you.
366
981000
2000
JE: Mirë, e kam për nder dhe privilegj të takohem me ty.
16:38
Thank you so much. (LM: Thank you so much.)
367
983000
2000
Faleminderit shumë. (LM: Faleminderit shumë)
16:43
JE: Thank you, Tony.
368
988000
2000
JE: Faleminderit shumë, Toni.
16:45
(Applause)
369
990000
3000
