ABOUT THE SPEAKER
Romina Libster - Medical researcher
Dr. Romina Libster investigates influenza and other respiratory viruses, searching for ways to most effectively keep viruses from spreading.

Why you should listen

Dr. Romina Libster is a staff scientist and assistant investigator at the National Scientific and Technical Research Council, in Buenos Aires, Argentina. She received her Doctor of Medicine, with High Honors from the University Of Buenos Aires School Of Medicine in 2004. She then completed her pediatric internship and residency at the “Pedro de Elizalde” Children’s Hospital in Buenos Aires, Argentina from 2004-2008. Upon completion of this training she began her Post-Doctoral Research Fellowship at the Fundación INFANT in Buenos Aires under the tutelage of a renowned investigator, Dr. Fernando P. Polack. Shortly after assuming her fellowship position, she began the Master in Clinical Effectiveness Program in Buenos Aires. She conducted a series of complex and innovative studies on respiratory viruses, with a special focus on influenza. Romina was invited by Dr. Kathryn M. Edwards to join the Vanderbilt Vaccine Research Program team at Vanderbilt University in 2009 where she is  leading a large clinical trial to determine the safety and immunogenicity of sequential rotavirus vaccine schedules. In 2013, Dr. Libster returned to her home country through a repatriation program from Fundación INFANT where she joined the faculty at the institution.

00:12
One of the first patients
I had to see as a pediatrician was Sol,
0
280
4780
uma linda bebê de um mês de vida
00:17
a beautiful month-old baby
1
5370
2780
00:20
who was admitted with signs
of a severe respiratory infection.
2
8150
3680
que deu entrada com sinais
de infecção respiratória grave.
00:23
Until then, I had never seen
a patient worsen so fast.
3
11830
4280
Até então, eu nunca tinha visto
um paciente piorar tão rápido.
Em dois dias, ela estava
ligada a um respirador
00:28
In just two days
she was connected to a respirator
4
16810
3000
00:31
and on the third day she died.
5
19810
2740
e no terceiro dia ela morreu.
Sol tinha coqueluche.
00:34
Sol had whooping cough.
6
22550
2750
00:37
After discussing the case in the room
and after a quite distressing catharsis,
7
25300
4710
Após discutir o caso, e passar
por uma catarse angustiante,
eu lembro que o meu
chefe-residente disse:
00:42
I remember my chief resident said to me,
8
30460
2049
"Tudo bem, respire fundo. Lave o rosto.
00:44
"Okay, take a deep breath. Wash your face.
9
32509
3341
Agora vem a parte mais difícil.
00:48
And now comes the hardest part:
10
36430
2620
Temos que falar com os pais."
00:51
We have to go talk to her parents."
11
39050
2410
Naquele momento, milhares de questões
vieram à minha mente:
00:54
At that time, a thousand questions
came to mind,
12
42150
3970
"Como uma bebê de um mês de vida
pode ser tão infeliz?"
00:58
from, "How could a one-month-old
baby be so unfortunate?"
13
46120
4330
01:03
to, "Could we have done
something about it?"
14
51200
2870
"Poderíamos ter feito algo a respeito?"
Antes de existirem as vacinas,
01:07
Before vaccines existed,
15
55400
2280
várias doenças infecciosas matavam
milhões de pessoas por ano.
01:09
many infectious diseases
killed millions of people per year.
16
57680
4870
Durante a epidemia de gripe de 1918,
01:15
During the 1918 flu pandemic
17
63100
3530
01:18
50 million people died.
18
66630
3150
50 milhões de pessoas morreram.
01:21
That's greater than Argentina's
current population.
19
69780
3280
Isso é mais do que a atual
população da Argentina.
01:25
Perhaps, the older ones among you
remember the polio epidemic
20
73060
4050
Talvez os mais velhos
se lembrem da epidemia de pólio
01:29
that occurred in Argentina in 1956.
21
77110
2920
que ocorreu na Argentina em 1956.
01:32
At that time, there was no vaccine
available against polio.
22
80240
3320
Naquela época, não existia
vacina disponível contra a pólio.
01:35
People didn't know what to do.
They were going crazy.
23
83560
2530
As pessoas não sabiam o que fazer,
estavam desoladas.
01:38
They would go painting trees
with caustic lime.
24
86090
2280
Elas pintavam as árvores com cal.
01:40
They'd put little bags of camphor
25
88370
1570
Colocavam bolsinhas de cânfora
01:41
in their children's underwear,
as if that could do something.
26
89940
3520
nas roupas íntimas das crianças,
como se isso pudesse ajudar.
Durante a epidemia de pólio,
milhares de pessoas morreram.
01:46
During the polio epidemic,
thousands of people died.
27
94080
4520
01:50
And thousands of people were left
with very significant neurological damage.
28
98600
4170
E milhares de pessoas ficaram
com danos neurológicos graves.
01:56
I know this because I read about it,
29
104650
2640
Eu sei disso porque li a respeito,
pois graças às vacinas,
minha geração teve a sorte
01:59
because thanks to vaccines,
my generation was lucky
30
107290
3790
de não viver uma epidemia
tão terrível quanto essa.
02:03
to not live through an epidemic
as terrible as this.
31
111080
2810
As vacinas são um dos grandes feitos
da saúde pública do século 20.
02:05
Vaccines are one of the great successes
of the 20th century's public health.
32
113890
4790
02:11
After potable water,
33
119210
2020
Depois da água potável,
02:13
they are the interventions
that have most reduced mortality,
34
121230
3610
elas são as intervenções
que mais reduziram a mortalidade,
02:16
even more than antibiotics.
35
124840
2130
mais até do que os antibióticos.
02:19
Vaccines eradicated terrible diseases
such as smallpox from the planet
36
127570
4990
As vacinas erradicaram doenças terríveis
do planeta, como a varíola,
02:24
and succeeded in significantly
reducing mortality
37
132590
3610
e conseguiram reduzir,
significativamente, a mortalidade
02:28
due to other diseases such as measles,
38
136200
2250
causada por outras doenças,
como o sarampo,
02:30
whooping cough, polio and many more.
39
138450
3330
coqueluche, pólio, entre outras.
Todas essas doenças,
estão dentro do grupo de doenças
02:34
All these diseases are considered
vaccine-preventable diseases.
40
142320
6880
consideradas imunopreveníveis.
O que isso quer dizer?
02:41
What does this mean?
41
149910
1760
02:43
That they are potentially preventable,
42
151670
3020
Que elas são potencialmente preveníveis,
02:46
but in order to be so,
something must be done.
43
154690
2970
mas para que isso aconteça,
algo deve ser feito.
02:49
You need to get vaccinated.
44
157660
2100
Você precisa se vacinar.
Eu imagino que a maioria,
se não todos que estão aqui hoje,
02:52
I imagine that most,
if not all of us here today,
45
160270
3700
02:55
received a vaccine
at some point in our life.
46
163970
3240
foi vacinada em algum momento da vida.
Agora, eu não tenho tanta certeza
se muitos de nós sabemos
03:00
Now, I'm not so sure that many of us know
47
168510
4450
03:04
which vaccines or boosters
we should receive after adolescence.
48
172960
4330
quais vacinas ou reforços deveríamos
receber depois da adolescência.
Você já se perguntou,
quem estamos protegendo
03:10
Have you ever wondered
who we are protecting
49
178710
4080
03:14
when we vaccinate?
50
182790
1850
quando nos vacinamos?
O que eu quero dizer com isso?
03:17
What do I mean by that?
51
185070
1630
Existe algum outro efeito
além da nossa própria proteção?
03:18
Is there any other effect
beyond protecting ourselves?
52
186700
4970
03:25
Let me show you something.
53
193050
2120
Deixe-me mostrar algo.
Imagine por um momento
03:27
Imagine for a moment
54
195760
2460
que nós estamos numa cidade
03:30
that we are in a city
55
198220
2010
03:32
that has never had a case
of a particular disease,
56
200230
3250
que nunca teve um caso
de uma doença em particular,
03:35
such as the measles.
57
203480
1700
como, por exemplo, o sarampo.
03:37
This would mean that no one in the city
has ever had contact with the disease.
58
205430
4860
Isso significa que ninguém na cidade
teve contato com a doença.
03:42
No one has natural defenses against,
nor been vaccinated against measles.
59
210290
4590
Ninguém tem defesas naturais
nem foi vacinado contra o sarampo.
Se um dia, uma pessoa com sarampo
aparecer na cidade,
03:47
If one day, a person sick with the measles
appears in this city
60
215680
4740
a doença não encontrará muita resistência.
03:53
the disease won't find much resistance
61
221000
3510
03:56
and will begin spreading
from person to person,
62
224510
2930
Ela começará a ser transmitida
de pessoa para pessoa
03:59
and in no time it will disseminate
throughout the community.
63
227440
4060
e, em pouco tempo, se disseminará
por toda a comunidade.
Depois de um tempo,
04:03
After a certain time
64
231810
1720
grande parte da população estará doente.
04:05
a big part of the population will be ill.
65
233530
3370
04:09
This happened when there were no vaccines.
66
237750
4160
Isso acontecia quando
não existiam as vacinas.
04:14
Now, imagine the complete opposite case.
67
242630
4480
Agora, imagine o caso totalmente oposto.
04:19
We are in a city
68
247420
2410
Estamos numa cidade,
04:21
where more than 90 percent
of the population
69
249830
2820
onde mais de 90% da população
04:24
has defenses against
the measles, which means
70
252650
2450
têm defesas contra o sarampo,
o que significa
04:27
that they either had the disease,
survived, and developed natural defenses;
71
255100
3739
que já tiveram a doença e desenvolveram
defesas naturais, sobreviveram
04:30
or that they had been
immunized against measles.
72
258839
3701
ou foram imunizados contra o sarampo.
Se um dia,
04:35
If one day,
73
263200
1610
uma pessoa com sarampo aparecer na cidade,
04:36
a person sick with the measles
appears in this city,
74
264810
3930
a doença encontrará maior resistência
04:41
the disease will find much more resistance
75
269230
3380
04:44
and won't be transmitted
that much from person to person.
76
272610
3490
e não será transmitida facilmente
de pessoa para pessoa.
04:48
The spread will probably remain contained
77
276780
3550
A disseminação, provavelmente,
será contida
04:52
and a measles outbreak won't happen.
78
280750
2930
e um surto de sarampo não ocorrerá.
04:57
I would like you
to pay attention to something.
79
285060
2700
Preste atenção a isto:
As pessoas que estão vacinadas
05:00
People who are vaccinated
80
288810
2680
05:03
are not only protecting themselves,
81
291490
2800
não estão apenas se protegendo,
05:06
but by blocking the dissemination
of the disease
82
294290
3610
mas ao impedir a disseminação da doença,
05:09
within the community,
83
297900
1640
dentro da comunidade,
05:11
they are indirectly protecting
the people in this community
84
299540
4680
estão, indiretamente, protegendo
as pessoas dessa comunidade
05:16
who are not vaccinated.
85
304220
2170
que não estão vacinadas.
Elas criam uma espécie de escudo protetor
05:18
They create a kind of protective shield
86
306690
2970
05:21
which prevents them from
coming in contact with the disease,
87
309660
3100
que as protegem de entrarem
em contato com a doença,
05:24
so that these people are protected.
88
312760
2620
assim elas estão protegidas.
05:28
This indirect protection
89
316670
3200
Esta proteção indireta,
05:31
that the unvaccinated people
within a community receive
90
319870
3500
que as pessoas não vacinadas
dentro da comunidade recebem
05:35
simply by being surrounded
by vaccinated people,
91
323370
4010
simplesmente por estar
cercadas por pessoas vacinadas,
se chama imunidade coletiva.
05:39
is called herd immunity.
92
327950
3060
Muitas pessoas na comunidade
05:45
Many people in the community
93
333130
2460
05:47
depend almost exclusively
on this herd immunity
94
335590
3500
dependem, quase que exclusivamente,
dessa imunidade coletiva
05:51
to be protected against disease.
95
339090
2660
para se proteger contra doenças.
05:54
The unvaccinated people you see
in infographics are not just hypothetical.
96
342760
3960
Essas pessoas não são
desenhos num gráfico.
Elas são nossos sobrinhos, nossos filhos
05:59
Those people are our nieces
and nephews, our children,
97
347260
3340
06:02
who may be too young
to receive their first shots.
98
350600
3370
que talvez sejam muito novos
para ter tomado as primeiras vacinas.
06:07
They are our parents, our siblings,
99
355100
2310
São nossos pais, nossos irmãos,
nossos conhecidos
06:09
our acquaintances,
100
357410
1430
06:10
who may have a disease,
101
358840
1870
que talvez tenham uma doença
06:12
or take medication
that lowers their defenses.
102
360710
3650
ou tomam remédios
que diminuem suas defesas.
06:18
There are also people who are
allergic to a particular vaccine.
103
366010
4070
Existem também pessoas que são alérgicas
a uma determinada vacina.
06:23
They could even be among us,
104
371350
2400
Elas poderiam estar entre nós,
06:25
any of us who got vaccinated,
105
373750
1860
qualquer um que foi vacinado,
06:27
but the vaccine didn't produce
the expected effect,
106
375610
3800
mas que a vacina não produziu
o efeito esperado,
porque nem todas as vacinas
são 100% efetivas.
06:31
because not all vaccines
are always 100 percent effective.
107
379410
4190
Todas essas pessoas dependem,
necessariamente, da imunidade coletiva
06:35
All these people depend
almost exclusively on herd immunity
108
383600
5200
06:40
to be protected against diseases.
109
388800
2560
para se proteger contra doenças.
06:44
To achieve this effect of herd immunity,
110
392390
4810
Para conseguir o efeito
da imunidade coletiva,
06:49
it is necessary that a large percentage
of the population be vaccinated.
111
397200
4670
é necessário que uma porcentagem
da população esteja vacinada.
06:54
This percentage is called the threshold.
112
402500
3070
Esta porcentagem é chamada de limiar.
06:57
The threshold depends on many variables:
113
405570
3760
O limiar depende de muitas variáveis:
07:01
It depends on the germ's characteristics,
114
409330
2410
das características dos germes;
das características das respostas
imunes geradas pela vacina.
07:03
and those of the immune response
that the vaccine generates.
115
411740
4300
Mas todas elas têm algo em comum:
07:08
But they all have something in common.
116
416040
2390
Se a porcentagem da população
em uma comunidade
07:10
If the percentage of the population
in a vaccinated community
117
418430
5210
que está vacinada estiver
abaixo desse número limiar,
07:15
is below this threshold number,
118
423980
3070
07:19
the disease will begin
to spread more freely
119
427050
4060
a doença começará a ser
transmitida livremente
07:23
and may generate an outbreak
of this disease within the community.
120
431110
4670
e poderá gerar um surto
dentro da comunidade.
07:27
Even diseases which were
at some point controlled may reappear.
121
435780
7080
Mesmo doenças que, de certo modo,
estavam controladas podem reaparecer.
07:36
This is not just a theory.
122
444550
2190
Isso não é só uma teoria.
07:38
This has happened,
and is still happening.
123
446740
2360
Isso aconteceu e ainda acontece.
07:43
In 1998, a British researcher
published an article
124
451050
4700
Em 1998, um pesquisador britânico
publicou um artigo
07:47
in one of the most important
medical journals,
125
455750
2780
em uma das mais importantes
revistas de medicina,
07:50
saying that the MMR vaccine,
126
458530
2620
dizendo que a vacina tríplice viral,
07:53
which is given for measles,
mumps and rubella,
127
461150
2730
dada contra o sarampo, caxumba e rubéola,
estava associada ao autismo.
07:55
was associated with autism.
128
463880
1870
07:57
This generated an immediate impact.
129
465750
2700
Isso gerou um impacto imediato.
As pessoas pararam de se vacinar
e de vacinar seus filhos.
08:01
People began to stop getting vaccinated,
and stopped vaccinating their children.
130
469010
4650
08:05
And what happened?
131
473680
1620
E o que aconteceu?
08:07
The number of people vaccinated,
132
475300
2680
O número de pessoas vacinadas,
08:09
in many communities around the world,
fell below this threshold.
133
477980
3980
em muitas comunidades pelo mundo,
caiu abaixo desse limiar.
08:13
And there were outbreaks of measles
in many cities in the world --
134
481960
3560
E houve surtos de sarampo
em várias cidades do mundo,
08:17
in the U.S., in Europe.
135
485520
2440
nos Estados Unidos, na Europa.
08:19
Many people got sick.

Várias pessoas adoeceram.
08:22
People died of measles.
137
490120
2780
Pessoas morreram de sarampo.
08:25
What happened?
138
493870
1480
O que aconteceu?
08:27
This article also generated a huge stir
within the medical community.
139
495350
4120
Esse artigo também gerou uma grande
comoção na comunidade médica.
Dezenas de pesquisadores começaram
a avaliar se isso era verdade.
08:32
Dozens of researchers began to assess
if this was actually true.
140
500020
3670
08:37
Not only could no one find
141
505130

Não apenas ninguém encontrou

Uma das principais preocupações
e desculpas para não se vacinar
09:04
One of the main concerns and excuses
for not getting vaccinated
147
532880
3900
são os efeitos colaterais.
09:08
are the adverse effects.
148
536780
2190
As vacinas, como outros medicamentos,
podem ter efeitos colaterais.
09:11
Vaccines, like other drugs,
can have potential adverse effects.
149
539770
5640
09:17
Most are mild and temporary.
150
545970
2510
A maioria é leve e temporário.
09:20
But the benefits are always greater
than possible complications.
151
548480
5990
Mas os benefícios são sempre maiores
do que as possíveis complicações.
09:27
When we are ill,
we want to heal fast.
152
555740

Quando estamos doentes,
queremos melhorar rápido.
Muitos de nós aqui,

09:38
we take anti-hypertensives

tomamos remédios para o coração.
09:41
we take cardiac medications.
156
569270

09:43
Why? Because we are sick
and we want to heal fast.
157
571450
3250
Por quê? Porque estamos doentes
e queremos melhorar rápido.
09:46
And we don't question it much.
158
574700

E não questionamos isso.
Por que é tão difícil pensar
em prevenção de doenças
09:48
Why is it so difficult
to think of preventing diseases,
159
576720
4430
e nos cuidarmos quando estamos saudáveis?
09:53
by taking care of ourselves
when we are healthy?
160
581890
3210
09:57
We take care of ourselves a lot
when affected by an illness,
161

Cuidamos de nós quando estamos doentes

Eu creio que a maioria dos que estão aqui,

I imagine most of us here,
163
592140
2670
10:06
remember the influenza-A pandemic
164
594810
3730
se lembra da pandemia de gripe A

10:30
People would line up

Formavam filas nas farmácias

Não sabíamos absolutamente nada.
10:39
We knew absolutely nothing.

Naquela época, além da minha
bolsa de estudos na Fundação INFANT,
10:41
At that time, in addition to doing
my fellowship at the Infant Foundation,
175
629650
4830
eu trabalhava como pediatra em domicílio

10:46
I worked as a home pediatrician
for a prepaid medicine company.
176
634740
3910
10:51
I remember that I started
my shift at 8 a.m.,
177
639670

Eu lembro que o meu
turno começava às 8h,
10:54
and by 8, I already had a list
of 50 scheduled visits.
178
642910

10:59
It was chaos;
people didn't know what to do.

Era um caos. Ninguém sabia o que fazer.
Me lembro dos tipos
de pacientes que eu examinava.
11:02
I remember the types of patients

180

11:07
The patients were a little older than
what we were used to seeing in winter,
181
655690
4360
Eram um pouco mais velhos do que aqueles
que costumávamos ver no inverno,
11:12
with longer fevers.
182
660050
2340
com quadros de febre mais prolongados.
E lembro de ter comentado
com o meu orientador,
11:14
And I mentioned that
to my fellowship mentor,
183
662390
3680
11:18
and he, for his part, had heard
the same from a colleague,
184
666070

e ele também ouviu de um colega,
11:21
about the large number
of pregnant women

sobre o grande número
de mulheres grávidas e adultos jovens

11:32
At that time, we set out to understand
what was happening.
189
680760

11:39
First thing Monday morning,
we took the car
190
687300
3120
Na segunda-feira, pegamos o carro
11:42
and went to a hospital
in Buenos Aires Province,
191
690420
2930
e fomos para um hospital
na Província de Buenos Aires
11:45
that served as a referral hospital
for cases of the new influenza virus.
192
693350
5670
que era referência para os casos
do novo vírus influenza.
Chegamos ao hospital, e estava lotado.
11:51
We arrived at the hospital;
it was crowded.
193
699020
2530
Toda a equipe de saúde estava vestida
com roupas de biossegurança, tipo NASA.
11:53
All health staff were dressed
in NASA-like bio-safety suits.
194
701550

11:57
We all had face masks in our pockets.

11:59
I, being a hypochondriac,
didn't breathe for two hours.

Imediatamente, começamos
a entrar em contato com pediatras
12:05

reaching out to pediatricians

12:09
from six hospitals in the city

717210

12:13
Our main goal was to find out
200
721440
3330
Nosso principal objetivo era descobrir
12:16
how this new virus behaved
in contact with our children,
201
724770
3890
como esse novo vírus se comportava
em contato com as nossas crianças,
12:20
in the shortest time possible.
202
728660
2580
Um trabalho desgastante.
12:23
A marathon work.
203
731240
2960
12:26
In less than three months,
204
734200
2870
Em menos de três meses,
12:29
we could see what effect
this new H1N1 virus had
205
737070
6090
conseguimos ver quais características
tinha este novo vírus H1N1
12:35
on the 251 children
hospitalized by this virus.
206
743160
6140
nas 251 crianças internadas
nos hospitais por causa dele.
12:41
We could see which children
got more seriously ill:
207
749520
3980
Podíamos ver quais crianças
que mais adoeciam:
12:45
children under four, especially those
less than one year old;
208
753500
3620
crianças abaixo dos quatro anos,
especialmente aquelas menores de um ano;
12:49
patients with neurological diseases;
209
757120

pacientes com doenças neurológicas;
crianças com doenças pulmonares crônicas.
